Refactor scoreboard teams handling

Filter more invalid scoreboard team packets (removing players from team
they doesn't belong to). Also greatly improve legacy prefix and suffix
conversion: team color now also properly affects suffix and prefix,
display line teams (teams with chat format only entries and no suffix)
now have special handling (prefix is split between prefix and suffix).
